​CDC’s National Center for Chronic Disease Prevention and Health Promotion recently awarded more than $200 million to public and private organizations through grant programs focused on populations hardest hit by chronic diseases. In addition to addressing leading risk factors for chronic disease, these awards provide new opportunities for chronic disease programs to receive technical assistance (TA) to achieve programmatic and infrastructure objectives using performance management, quality improvement, and workforce development.

 

The Public Health Foundation (PHF) is part of the TA Provider Network for several of these programs. Through this TA, PHF can help grantees build and strengthen interdisciplinary teams and partnerships; establish health system improvements and community supports; prioritize interventions and implement action steps; identify and address barriers to successful implementation; and monitor and evaluate progress toward goals.


Grantees of the following programs can request TA at no cost:

Although not free, grantees in several other programs can also access this TA by using grant funds in a direct agreement with PHF. These include grantees of State and Local Public Health Actions to Prevent Obesity, Diabetes, and Heart DiseaseState Public Health Actions Enhanced Awards, Programs to Reduce Obesity in High-Obesity Areas, and A Comprehensive Approach to Good Health and Wellness in Indian Country.
