PHF Announces 2012 "I'm Your Community Guide!" Contest Winners

Today, the Public Health Foundation (PHF) is pleased to announce the winners of the "I'm Your Community Guide!" Contest, which is designed to identify model practices for implementing the evidence-based strategies outlined in the Guide to Community Preventive Services (Community Guide).
 
The Michigan Cancer Consortium (MCC) has been awarded first place for their story, “The MCC Challenge.”  MCC utilized findings and recommendations from the Community Guide to implement a breast, cervical, and colorectal cancer screening promotion program among their own employees, which impacted approximately 7,000 employees and dependents as of March 2011 when the program was first launched.  Learn more about their success with the Community Guide.
 
“The Community Guide and Personalized Educational and Experiential Modules for Diabetes Management” from the Austen BioInnovation Institute in Akron, Ohio came in second place.  Their use of Community Guide recommended strategies was evident in developing, implementing, and evaluating a diabetes management program within their Center for Community Health Improvement. Third place went to Central Michigan District Health Department (CMDHD) for their entry, “Together We Can! Create a Healthier Community.”  CMDHD utilized the Community Guide to identify evidence-based initiatives to apply in their region to address the eight health priorities established for their wellness program.  Learn more about their success with the Community Guide.
