Below are two assessment tools, one designed for state health departments and the other designed for local health departments. Both tools are based on a model for performance management (PM) that follows a quality improvement (QI) cycle in which organization performance information and population outcome data are fed back into the system to improve future decisions. In this model, the main components of the Performance Management System are Plan-Do-Check-Act (PDCA). The assessment questions are based on key types of alignment and select practices needed for a robust PM System.
