Population Health Driver Diagram to Increase Use of Oral Health Care

Overview

With funding from the Office of Disease Prevention and Health Promotion of the U.S. Department of Health and Human Services, the Public Health Foundation (PHF) led an effort to develop a Population Health Driver Diagram to Increase Use of Oral Health Care. A population health driver diagram identifies primary and secondary drivers of a population health goal, and helps to promote alignment of activities across sectors to address community health challenges, such as increasing the use of oral health services. PHF has created a tool to help develop population health driver diagrams.

The Population Health Driver Diagram to Increase Use of Oral Health Care was developed by PHF after reviewing the literature and receiving input from leaders in quality improvement (QI) and oral health care subject matter experts. This driver diagram contains primary and secondary drivers to “increase the proportion of children, adolescents, and adults who used the oral health care system in the past year,” which is a Healthy People 2020 Leading Health Indicator. In May 2014, an interdisciplinary team of experts in QI, oral health, epidemiology, and public health practice gathered to refine this Population Health Driver Diagram to Increase Use of Oral Health Care. PHF is currently seeking sponsors to help test and further refine the driver diagram for potential use in communities throughout the United States.
