SIPOC+CM+CE Collection Form

Overview

The SIPOC+CM+CE Collection Form (SIPOC+CM+CE), a variation of the SIPOC+CM Diagram, identifies major elements of the core processes of an organization: Suppliers, Inputs, Process, Outputs, Customers, Constraints, and Measures. For this variation, Competitors and Expectations are included for additional depth of analysis. Developed by Public Health Foundation (PHF) Senior Quality Advisor Jack Moran, SIPOC+CM+CE creates a high-level map of cross-functional activities and can help identify performance measures for the process under analysis. This is a more advanced quality improvement (QI) tool and supplements the many tools in the 2012 Public Health Quality Improvement Encyclopedia.
