Developed by Public Health Foundation (PHF) Senior Quality Advisor Jack W. Moran and PHF performance improvement experts Les Beitsch and Annie Vosel, this tool supports the Plan-Do-Check-Act (PDCA) approach to quality improvement by providing a structured documentation process.
