The Community Stakeholder Services Map assists a community health improvement team in identifying services affiliated with a community health issue. This quality improvement (QI) tool helps develop a visual representation of where services are physically located in relation to each other.
Developed by Public Health Foundation (PHF) Senior Quality Advisor Jack W. Moran, the tool makes the service network come to life, showing the variety of locations a customer may need to visit to obtain services.
