Mistake Proofing

Mistake Proofing (also known as poka-yoke) helps prevent errors from becoming defects. Originally developed by Shigeo Shingo in the 1960s, this tool uses countermeasures to alert users before errors occur or blocks incomplete actions (e.g., preventing form submission if a required field is missing). Often paired with flowcharting and analysis, it helps safeguard critical processes. 

Developed by Public Health Foundation (PHF) Senior Quality Advisor John W. Moran and PHF quality improvement expert Sonja Armbruster, this tool supports teams in building more error-proof systems.
