While public health organizations and their community partners strive to remain nimble in the context of a changing health system, the Public Health Foundation (PHF) has developed new performance improvement services that leverage partnerships to achieve population health goals. Two new services just released by PHF respond to the need for public health and health care organizations to use collaborative solutions. Both new services use quality improvement tools and performance management frameworks to guide decision-making and optimize results.
Driver Diagram Development for Community Health Challenges is a facilitated intervention to guide a team in mapping primary and secondary drivers to address a specific community health challenge. PHF has led initiatives using this approach with Antibiotic Stewardship and Oral Health Care.
