Getting the Community Health Improvement Plan Off the Shelf and Into Practice

Overview

A community health improvement plan (CHIP) identifies actions that can be taken to improve the community’s health. This is an important step, but many communities find themselves challenged to move from the CHIP to action. In this white paper from Public Health Foundation (PHF), authors Senior Quality Advisor Jack Moran, Performance Improvement Expert Sonja Armbruster, and PHF Board member Lee Thielen share how quality improvement tools can be used to help get the CHIP off the shelf and into practice. The creation of a Force Field Analysis, Cause and Effect Diagram, and Solution and Effect Diagram can be effective in generating enthusiasm and progress in implementing a CHIP.
