Through this initiative,
Our Lady of the Lake Regional Medical Center in Baton Rouge, LA and
Bon Secours Maryview Medical Center in Portsmouth, VA worked with public health and other community stakeholders, explored evidence-based strategies, and took specific actions to address HIV/AIDS and obesity and improve the health of their communities. Throughout the initiative, these hospitals drew on evidence-based resources, such as
The Community Guide (The Guide to Community Preventive Services), and quality improvement methods and tools, including a
population health driver diagram framework, to address priority health needs in their communities.
During this webinar, representatives from Our Lady of the Lake Regional Medical Center and Bon Secours Maryview Medical Center shared their experiences with the initiative, including successes and lessons learned. Methods and tools that supported the initiative, including The Community Guide and the population health driver diagram framework, were also featured.
