Featuring two communities participating in the Using The Community Guide for Community Health Improvement pilot initiative, this archived webinar offers an overview of this pilot initiative and highlights the stories of action being taken in Portsmouth, VA and Baton Rouge, LA.
Through this initiative, Bon Secours Maryview Medical Center in Portsmouth, VA and Our Lady of the Lake Regional Medical Center in Baton Rouge, LA worked with public health and other community stakeholders, explored evidence-based strategies, and took specific actions to address obesity and HIV/AIDS and improve the health of their communities. Throughout the initiative, these hospitals drew on evidence-based resources, such as The Community Guide (The Guide to Community Preventive Services), and quality improvement methods and tools, including a population health driver diagram framework, to address priority health needs in their communities.
During this webinar, representatives from Bon Secours Maryview Medical Center and Our Lady of the Lake Regional Medical Center shared their experiences with the initiative, including successes and lessons learned. Methods and tools that supported the initiative, including The Community Guide and the population health driver diagram framework were also featured.

Four hospitals have been engaged in pilot testing this framework and have worked with community stakeholders to address health priorities ranging from behavioral health to obesity to HIV/AIDS.
