From 2011–2013, The Public Health Foundation (PHF) conducted the “I’m Your Community Guide” Contest, which was designed to identify model practices for implementing the evidence-based strategies outlined in The Guide to Community Preventive Services (The Community Guide). The success stories that PHF gathered came from organizations across the nation:
- Austen BioInnovation Institute in Akron, Ohio (2012) used The Community Guide to develop, implement, and evaluate a diabetes management program that resulted in weight loss and BMI decrease in 14 patients
- Central Michigan District Health Department (2012) used The Community Guide to complete and approve their 2012 Together We Can Community Health Assessment and Health Improvement Plan that improved the overall health of the 196,000 residents living in the district
- Duval County Health Department in Florida (2011) used vaccination findings and recommendations from The Community Guide to increase the 4:3:1:3:3:1 series immunization rates among their two-year-old population by 15% in two years
- Maricopa County Department of Public Health in Arizona (2013) used The Community Guide to develop a database of 800 itemized public health-related evidence-based practices and a five-year community health improvement plan for addressing priority health issues in the county
- Michigan Cancer Consortium (2012) used The Community Guide to increase breast, cervical, and colorectal cancer screening rates among their 7,000 employees and employee dependents
- Sedgwick County Health Department in Kansas (2013) used The Community Guide to implement Walktober, a community-wide campaign to increase physical activity in order to decrease risk for diabetes, cardiovascular disease, and other chronic diseases
- Sedgwick County Health Department's Office of Health Promotion in Kansas(2013) used evidence-based recommendations from The Community Guide for smoking bans and restrictions to achieve an increase in smoke-free multi-unit dwellings in their community
- St. James-Santee Family Health Center in South Carolina (2011) used The Community Guide to implement a breast and cervical cancer screening promotion program, which increased Pap smear and mammogram rates by 10% and decreased missed appointment rates by 30%
- Whidbey General Hospital in Washington (2011) used tobacco interventions from The Community Guide to achieve a tobacco quit rate of 35-40% in one year
