Build Your Skills and Get Ready for Community Health Improvement Week 

 

Join the Public Health Foundation (PHF) in celebrating Community Health Improvement Week this June 4-10, 2017. Explore resources to support community health improvement from the Association for Community Health Improvement and register for one of the nearly 150 community health trainings available through the TRAIN Learning Network.

 

Resources for Community Health Improvement
  • Solving Population Health Problems through Collaboration is a new book that provides insights from more than 50 experts in population health to help navigate the partnerships between public health departments, hospitals, and their community partners to improve health. It offers step-by-step collaboration advice for population health executives and staff during this time of rapid change in healthcare and public health.
  • The Guide to Community Preventive Services (The Community Guide) is a collection of evidence-based findings of the Community Preventive Services Task Force. The Community Guide helps select interventions to improve health and prevent disease in states, communities, community organizations, businesses, healthcare organizations, and schools.
  • The CDC Community Health Improvement Navigator (CHI Navigator) is a website for people who lead or participate in community health improvement work within hospitals and health systems, public health agencies, and other community organizations. The CHI Navigator is a one-stop-shop that offers community stakeholders expert-vetted tools and resources.
  • The Community Health Assessment Toolkit offers a nine-step pathway for conducting a community health assessment and developing implementation strategies.
  • Health and Well-Being for All: Meeting-in-a-Box is a tool for accelerating learning about social determinants of health. Use this interactive resource to facilitate conversations with colleagues, students, and partners to create change agents, collaborate across sectors, and address the core issues affecting health in your community.

     

     
    Training Spotlight:
    "Community Health Assessment and Improvement Planning" Training Plan
     
    Based on practical materials from the Community Tool Box, these eight short online trainings provide practical guidance for conducting community health assessments and developing and implementing community health improvement plans.

    New Resources to Support Community Health Improvement Efforts

    Interested in learning how hospitals, health departments, and other community stakeholders are taking action on community health improvement activities? The recent webinar features two communities participating in the Using The Community Guide for Community Health Improvement pilot initiative and offers insight into how coalitions of community stakeholders led by WellSpan Health (York, PA) and INTEGRIS Health (Oklahoma City, OK) have been using the population health driver diagram framework to implement community health improvement activities to address behavioral health needs. More information about this project and the archive can be accessed from the event webpage or on TRAIN
